City Clerk's Office

  • All land recordings - Recordings can be done in person Monday - Friday 8:30 AM - 4:00 PM or can be mailed to Warwick City Hall, City Clerk's Office, 3275 Post Road, Warwick, RI 02886. Please include a self-addressed stamped envelope for your documents to be returned. *Electronic Recordings are not accepted after 3:00 PM*
  • Vital Records - Birth, Death and Marriage Certificates are issued Monday – Friday 8:30 AM to 3:30 PM. 
    Marriage Licenses are issued Monday – Friday 8:30 – 12:00PM and 1:00 PM– 3:00 PM by appointment ONLY 
    Please call 401-738-2006 Ext 1 to make an appointment and confirm you have the proper documents. 
         ***We accept Cash, Check or Money Order ONLY***    
  • Mail requests are being accepted for Birth, Marriage and Death Certificates. Please include a copy of your driver’s license, a check made payable to the City of Warwick, and a self-addressed stamped envelope with your completed, signed application. Applications can be mailed to City of Warwick, Vital Records, 3275 Post Road, Warwick, RI 02886.
  • Municipal Court  is held on Wednesdays at 8:30 in the Council Chambers. Payments are accepted Monday - Friday 8:30 AM - 4:00 PM.
  • Probate Court - The Office is open Monday - Friday 8:30 AM - 4:00 PM for in person filings. You may also mail petitions to Warwick City Hall, Probate Court, 3275 Post Road, Warwick, RI 02886.
  • If you require a trade name certificate, please download the form, include a self-addressed stamped envelope and mail with a check payable to City of Warwick for $10, City Clerks, 3275 Post Road, Warwick, RI 02886. Trade Name Certificates can be done in person Monday - Friday 8:30 AM - 4:00 PM
  • Dog License applications can be mailed to City of Warwick, City Clerk – Dog License, 3275 Post Road, Warwick, RI 02886. Please include the application, a check for $10 (made payable to the City of Warwick) the rabies certificate and a self-addressed stamped envelope. Dog Licenses can be done in person Monday - Friday 8:30 AM - 4:00 PM, please complete the application and bring the current rabies certificate.

The City Clerk's office is the custodian of the city seal and all official documents and records of the city dating back to 1642. The mission of the City Clerk's office is to make as much public information available to the public in an easily accessible format and to protect the integrity of our historical records.
