COVID-19 Vaccine Information
The City Warwick is currently booking appointments for upcoming Warwick COVID-19 Vaccine Clinics. This clinic is open for Warwick residents aged 65 years or older only.
SCHEDULING YOUR VACCINE VIA SMARTPHONE OR COMPUTER:
Warwick residents 65 or older that are technologically able can register themselves for open appointments on one of the following clinic dates:

PLEASE NOTE THE FOLLOWING:
- Anyone who registers that is not 65+ or a resident of Warwick will have their appointment cancelled. In the coming days, additional clinic dates will be added to this website.
- Clinics will adhere to all COVID-19 safety guidelines and best practices. Upon arrival, individuals who enter the clinic will undergo medical screening and will be required to wear a mask. If residents are sick or in quarantine for COVID-19, they are encouraged to stay home and book a new appointment.
- Anyone who would like to schedule a vaccine but does not have access to a computer may call Senior Services at 401-468-4070. Individuals may speak with a staff member or leave a message to be contacted.

UNABLE TO LEAVE YOUR HOME FOR A VACCINE:
If you are unable to leave your home to receive vaccine, or if you care for someone who cannot leave home, complete the form at www.bit.ly/homeboundvax to let the Department of Health know. If you need help completing this form, call the Rhode Island Healthy Aging Help Desk (Point) at 401-462-4444. We will do everything we can to get a vaccine to you.
