The State of Rhode Island’s current eligibility list for COVID-19 vaccination includes individuals age 18+ for Moderna and Johnson & Johnson, and age 16+ for Pfizer.
Free COVID-19 vaccinations are available to individuals age 18+ for Moderna and Johnson & Johnson, and age 16+ for Pfizer at three state-run vaccination sites:
- 100 Sockanosset Cross Road, Cranston 02920
- Dunkin’ Donuts Center, 1 LaSalle Square, Providence 02903
- 1400 West Main Road Middletown RI 02842
Eligible individuals may register to be vaccinated at one of the State-Run sites by visiting www.vaccinateri.org
People who cannot register online can get help by calling the automated line at 844-930-1779.
As supply allows, new COVID-19 vaccination appointments at the State-run vaccination sites will be made available at the following times:
- Beginning at approximately 9 a.m. on Tuesdays
- Beginning at approximately 5 p.m. on Fridays
Additional appointments may also be posted throughout the week as they become available.
Some pharmacies including Walgreens, Walmart, Stop and Shop, and CVS have some appointments available for individuals age 18+ for Moderna and Johnson & Johnson, and age 16+ for Pfizer.

Lifespan and Care New England have received a limited supply of COVID-19 vaccine to administer to patients according to Rhode Island Department of Health eligibility criteria. Lifespan and Care New England are providing COVID-19 vaccinations by appointment only and will not take any walk-ins at the clinics.
UNABLE TO LEAVE YOUR HOME FOR A VACCINE:
If you are unable to leave your home to receive vaccine, or if you care for someone who cannot leave home, complete the form at www.bit.ly/homeboundvax to let the Department of Health know. If you need help completing this form, call the Rhode Island Healthy Aging Help Desk (Point) at 401-462-4444. We will do everything we can to get a vaccine to you.
COVID-19 Vaccine Record Lookup
Rhode Islanders can now get a copy of their COVID-19 vaccine record through portal.ri.gov/VaccineRecord. From this website, you can search for and print proof of your COVID-19 vaccination. If you need a second dose of vaccine, this record will tell you when you are eligible to get your second dose. Please note that only people who have received their COVID-19 vaccination in Rhode Island will be able to look up their vaccination record online. If you are having trouble finding your vaccination record or are not able to go online, you can call 401-222-8022 for help.
